Doc's to prepare before Labor

...hi mga mommies / mom-to-be,

here it goes...aside from our Hospital bag: clothes ni mommy ni baby and ni daddy and other things to bring and ofcourse money or card don't also forget
Goverment Documents and ID's (include pen)..

Here it follows:

1. SSS
            > If you are a member and highest contribution ka mommy (like me) here dapat yung makukuha mu in advance to your employer: (it means 1month advance as per SSS policy).
            > 32,000.00php is the new Maternity benefit (based on new Table of contribution 2014) and it is under NSD or yung tinatawag na NORMAL Delivery; just in case na CS ka the amount you can get from SSS is 41,900.00php pero since hindi mo naman malalaman agad na CS ka if wala ka namang other health condition usually 32,000.00 lang muna..yung 9,900.00php na additional is pagbalik mu na ng office.

But if may health condition ka na no choice kundi CS, you need to submit Health requirements and Doctors recommendation, pwede din daw ang Medical history to prove your claim in your office so they can release 41,900.00php na agad...para di ka na mag re-imburse.

If hindi ka naman highest contributor ask your HR na lang how much you can get;

then if voluntary ka naman, you can go to nearest SSS brach near you then sila na yung magcompute and even issue a cheque via delivery to your home address na nakarecord sa kanila so better update your information para less hassle.

2. Philhealth
              > MDR - you can get your MDR (Member Data Record) at any Philhealth branches.  (update your MDR as much as possible like if you are single to married; or change of surname;address;contact number and so on).

If you are not a Philhealth member and your hubby is, then your hubby can get his MDR at any philhealth, then dapat dependent ka nya para pwede mong magamit.
            > CF1 (Claim Form 1) - you can request it to your company before hand, para masign nila yung sa may employer part..request two copies for you and for your baby.

If si hubby ulet ang may philhealth same procedure lang request sya ng two copies sa company nya...pwede namang blank details muna the ifill-up na lang..ang importante dun yung employer part. para hindi na magpapabalik balik sa company.

            > Certificate of Contribution - you can request it to your company. request 2 copies na din for sure lang.

Same procedure pa din if si hubby ang may philhealth. HR naman po ang gumagawa nun.. :)

Ok here is the other possible question:
>if both of you is Philhealth member -  you can both use your philhealth but not in the same purpose..get it..example si mommy can use her philhealth benefit for DELIVERY/ LABOR, then si Hubby can use his Philhealth for BABY - hospitalization/newborn screening etc. Then sa requirements kelangan nyo both magrequest na MDR; CF1 and certificate of contribution.
dont forget your Philhealth ID and ballpen (black).

>if Voluntary member - same requirements lang; certificate of contribution and CF1 can be requested at Philhealth offices near you.

if di ka member - ofcourse you cannot avail.. if not continously ang contribution payment mo naman..better go to Philhealth office near you and inquire habang may time pa..

NOTE: Pregnant is always a Priority like senior and PWD..so no need to fall in line.

3. Others
             >  for less hassle lang po bring na din the following:
                      > Marriage Contract (original and photocopy) - for marriage only
                      > MAT 2 - if voluntary member ka sa SSS; if member you can also bring if you want.
                      > Other Government ID's; Company ID and other valid ID
                      > Ballpen - Black and Blue pen (make sure na sumusulat naman)
                      > If may BIRTH CHEAT SHEET ka,,bring na din para hindi hassle yung pagfifill-up for Birth Certificate ni Baby..
                      > Prepare na din Mother's Infornation sheet para less question na sa hospital.
                      > Credit Card / Debit Card / Money (Bills & Coins) - for Hospital bill/ other na bibilhin; foods; parking etc.
                      > Don't forget your Cellphone with LOAD and full charge (bring charger na din for sure_) and CAMERA :)
